We start with your concerns and experiences of health to understand where you are in your health journey. We’ll put your questions on the table and address any outstanding information gaps, plus identify any requirements for information gathering. Shared goal definition is at the heart of this session.
Together, we answer: Where you’re starting from, where you’re trying to go, and what supporting information will be required to develop a targeted plan.

Health mapping considers not only where you’re aiming to go in your health journey, but how you’ll get there. It’s the process of putting a step-by-step plan in place — including key milestones and recommendations — based on what matters most to you. We identify the things that get in the way so you can keep the momentum going.
Together, we discover: The steps required to work toward your health goals and ways to know when you get there.
